Quick Suggestions:

• It is recommended to use this medicine in the prescribed manner. However, anti-reflux medicines should preferably be taken in the morning on an empty stomach or with food for optimum effectiveness.
• You can make certain lifestyle modifications like preventing excess intake of foods that cause acidity, like carbonated drinks and oily/fried food, regular exercise, and maintaining a healthy weight.
• It is also advised to limit alcohol intake and quit smoking for better treatment of your acidity.
• It is advised to reduce stress and get good sleep each night.
• Inform your doctor if you are using this medicine as a continuous treatment of acidity for more than 4 weeks or if the symptoms are persistent even after taking the medicine.
• This medicine can hamper the actions of other medicines taken simultaneously. Inform your doctor if you are using any other medicines.

Indication

Sompraz 40 tablet is used for the treatment of acid reflux disease, stomach and intestinal ulcers, and Zollinger-Ellison syndrome. It contains esomeprazole as its active ingredient.

Pharmacology

Esomeprazole is a proton pump inhibitor (PPI) that works by specifically inhibiting the H+/K+ ATPase enzyme in the gastric parietal cells of the stomach. This enzyme is responsible for the final step in the production of gastric acid. By inhibiting this enzyme, esomeprazole reduces the production of stomach acid, which can help in the treatment of conditions such as gastroesophageal reflux disease (GERD), peptic ulcers, and other acid-related disorders.

Esomeprazole is the S-isomer of omeprazole, which means it is a single optical isomer of omeprazole. This single isomer formulation provides better acid control than racemic PPIs (those containing both S and R isomers), making it more effective in acid suppression and providing longer-lasting effects.

Dosage & Administration of Esoral Mups 20mg Tablet

Adult Dose:

GERD Without Erosive Esophagitis: 20 mg orally once a day for 4 weeks; consider an additional 4 weeks of treatment if symptoms do not completely resolve in the first 4 weeks.

GERD With Erosive Esophagitis: 20-40 mg orally once a day for 4-8 weeks. If oral therapy is not possible, 20-40 mg intravenously (IV) once a day for up to 10 days, then switch to oral administration once the patient can swallow.

Maintenance: 20 mg orally once a day for up to 6 months.

Risk Reduction of NSAID-Associated Gastric Ulcer: 20-40 mg orally once a day for up to 6 months.

NSAID-Induced Gastric Ulcer: 20 mg orally once a day for 4-8 weeks.

Zollinger-Ellison Syndrome: Initially 80 mg orally divided every 12 hours, adjust regimen to efficacy, up to 240 mg orally once a day or 120 mg orally every 12 hours.

Elderly: No dosage adjustment is needed.

Hepatic Impairment:

• Mild to moderate (Child-Pugh A/B): No dosage adjustment is required.
• Severe (Child-Pugh C): Not to exceed 20 mg/day.

Child Dose:

GERD Without Erosive Esophagitis:

• 1-12 years: 10-20 mg orally once a day for up to 8 weeks.

12 years: 20-40 mg orally once a day for up to 8 weeks.

GERD With Erosive Esophagitis (Healing):

• 1 month to 1 year (weight-dependent):
  • <1 month: Safety and efficacy not established.
  • 1 month to 1 year (3.5 kg): 2.5 mg orally once a day for up to 6 weeks.

3.5-7.5 kg: 5 mg orally once a day for up to 6 weeks.

7.5 kg: 10 mg orally once a day for up to 6 weeks.

• 1-12 years:
  • <20 kg: 10 mg orally once a day for 8 weeks.

20 kg: 10-20 mg orally once a day for 8 weeks.

12 years: 20-40 mg orally once a day for 4-8 weeks.

Maintenance: 20 mg orally once a day for up to 6 months.

Renal Dose:

Renal impairment: No dosage adjustment is needed.

Interaction

Sompraz 40 tablets can interact with other medications, affecting their effectiveness or how Sompraz 40 tablet works. It is crucial to inform your doctor about all the medicines you are taking, including prescribed, non-prescribed, herbal medicines, dietary supplements, and other treatments for any disorders.

Particular caution should be taken if you are using medicines for heart-related disorders like digoxin, brain-related diseases, anti-epileptic medicines like phenytoin, anti-fungal medicines, antipsychotic medicines, etc. Sompraz 40 tablets may decrease the effectiveness of Anti-HIV medicines if taken together.

Additionally, the blood-thinning action of Warfarin may increase when used with Sompraz, leading to a risk of bleeding. Medications like cisapride (used for constipation), clopidogrel (blood-thinner), and tacrolimus (immunosuppressants) should also be used cautiously with Sompraz 40 tablets. Always consult your doctor for guidance to avoid potential interactions and ensure safe and effective treatment.

Contraindications

• If you are allergic to esomeprazole or any other ingredients of Sompraz 40 tablet.
• If you are taking the anti-HIV medicine Nelfinavir.

Pregnancy & Lactation

Esomeprazole is FDA-approved for use during pregnancy and falls under pregnancy category B, indicating that animal studies have not shown any risk to the fetus, but there are no adequate and well-controlled studies in pregnant women.

However, there is no available data on the excretion of Esomeprazole into human milk for nursing mothers. Therefore, caution should be exercised, and nursing mothers should consult their doctors before using Esomeprazole to assess the potential risks and benefits. The decision to use the medication should consider the importance of breastfeeding and the potential impact on the breastfed infant.

Precaution & Warnings:

1. You are allergic to the active ingredient esomeprazole or any other medicine.
2. You experience unexplained weight loss, frequent vomiting, difficulty in swallowing, and blood vomiting.
3. You are undergoing long-term treatment with this medicine.
4. You are undergoing treatment for a bacterial infection caused by H. pylori and using an antibiotic called Clarithromycin.
5. You develop stomach infections after using Sompraz 40 tablets.
6. You have a vitamin B12 deficiency, as the use of Sompraz can result in very low levels of Vitamin B12 during prolonged therapy.
7. You have a magnesium deficiency, as it can lead to very low levels of magnesium characterized by weakness, fits, and irregular heartbeats.
8. You are an elderly patient on long-term therapy, as it increases the risk of fracture of the hip, wrist, and spine bones.
9. You have a skin disorder called subacute cutaneous lupus erythematosus, characterized by a rash on the areas of the skin exposed to the sun.
10. You are using anti-HIV medicines like Atazanavir or Nelfinavir.
